Required:

• Must have a National Provider Identifier (NPI)

• Must have malpractice insurance
• Must get credentialed with insurance (takes up to 120 days after obtaining a WA license)
• Must be available immediately after being credentialed with insurance.

• Medical insurances require that individuals who graduated less than one year ago have a credentialed mentor (provided by the clinic).

• Individuals already on OPT or H1b valid through Oct. 2009, or E3 (Australia) visas are welcome to apply. 

• Must have a clean record. No criminal record & no malpractice suits
• Must be willing to allow a background check and/or credit check upon request 

Salary Range:

Year 1: $30,000 - $36,000 based on experience. 

Year 2: Up to $45,000 (25% salary increase) based on productivity. Annual raises and/or bonuses in subsequent years based on productivity.
